How can I get immunizations for myself or my child?
We recommend going to your medical provider. If you do not have a medical provider, you can come to the TIKE van for immunizations [schedule]. Children under 19 can get MMR immunizations at the Family Planning Clinic.
How can I make sure my child will meet my school's immunization requirements?
All children need to have an up-to-date copy of immunization records to be enrolled in, and to attend, school. If your child's immunizations need to be updated and you bring written proof of an appointment in the next 20 days, you may temporarily enroll your child pending receipt of the required immunizations. Your doctor or health clinic can provide the DHMH 896 Form, or a computer-generated form, for you to take to the school.[pdf]
