The Childhood Asthma Program provides care coordination and home visiting to high-risk children with asthma, who are referred by pediatricians, school staff or others. The Program works to ensure that children with asthma are connected to a medical home that can provide appropriate health care and information to parents to help manage their child’s asthma. The Program is able to conduct home visits to assess for environmental triggers that may exacerbate the asthma, and to provide tools such as mattress covers and air filters that can lessen the problem. Any child age 0-6 residing in Baltimore City who has been diagnosed with asthma by a health care provider is eligible for the Childhood Asthma Program.
Home visiting services are available to:
- Children ages 0-6 who are
- Baltimore residents and who have been
- diagnosed with asthma by a healthcare provider.
If your child meets these requirements, please call 410-396-1273 to enquire about enrolling in the Asthma Program.
Asthma Action Plan
An asthma action plan is a form that your health care provider fills out telling you how to manage your child's asthma:
- You can download the Asthma Action Plan here.
- Once you've printed it out, have it filled out by your provider.
- Keep a copy for yourself, and use it as a guide for managing your child's asthma
- You should also give a copy with your child's school nurse or health aide so your child's asthma can be managed at school
Need help getting your asthmatic child the care he or she needs in school?
1. Print out a School Health Asthma Action Plan (click here to download) and have it filled out by your pediatrician. This plan is a guide to managing your child's asthma.
